1. I consent and authorize Practical Life Coaching, LLC (Releasee) to use my likeness in any audio, photograph, video or other digital media (“Recordings”) taken or to be taken on Saturday, February 8, 2024 (Date) during a channeling event (Event) in any and all of its publications, including print or web-based publications.
2. I irrevocably authorize Practical Life Coaching, LLC (Releasee) to copy, edit, enhance, crop, or otherwise alter any or Recordings for use in their publications. I also waive any rights for approval or inspection of any Recordings.
3. I understand and agree that all Recordings are the property of Practical Life Coaching, LLC (Releasee) and will not be returned to me.
4. I acknowledge that I am not entitled to any compensation or royalties with respect to the use of the Recordings.
5. I agree to release and forever discharge Practical Life Coaching, LLC (Releasee) and its affiliates, successors and assigns, officers, employees, representatives, partners, agents and anyone claiming through them, in their individual and/or corporate capacities from any and all claims, liabilities, obligations, promises, agreements, disputes, demands, damages, causes of action of any nature or kind, known or unknown, which I, and anyone claiming on behalf of me, may have or claim to have against Releasee in connection with this Release.
6. I have carefully read and fully understand all the provisions of the Photo Release Form and am freely, knowingly and voluntarily signing.
7. My online submission of this form verifies that I understand and agree to the provisions of this release form in perpetuity.
